This is the nicest restaurant in Vernal, and the wait times reflect that. Reservations are a must and I was grateful for my takeout order when I saw how hectic the vibe was inside.

Good pizza! Although a bit pricey and not my favorite crust style, we highly recommend this place. I wish they had ranch dressing—but the complimentary cheese bread is great and the pizza is good.

Fried calamari was some of the best I’ve ever tasted.
Wood fired margarita pizza was also top notch!
Great selection of cold beers and local-ish wines available. I’ll deff return to try a pasta dish and a desert.

Home made Mozzarella is awesome!
Chewy, rich flavor, tasty cheese!
Caprase is best way to enjoy Mozzarella.
And Marinara is also very good, simple but tasty pizza to enjoy pizza craft.
But my daughter ordered one, Margherita is also good way to enjoy both Mozzarella and Pizza.
The other side, pasta is also tasty.
Seefood pasta is originally spicy one.
But they’ve offered to adjust to mild taste for my kids.
Even mild source, it was very tasty.
Absolutely, come again!

The restaurant was conveniently located next to the hotel where I was staying so I ordered carryout. The pistachio pizza was incredible,so flavorful. Just the right amount of char on the crust that you’d expect from a wood-fired pizza. The sausage had just the right amount of spice. I also tried the caprese salad and it, too, was delicious. Fresh made-in-house mozzarella and balsamic. The tomatoes tasted like they had just been picked out of the garden. If you’re in Vernal, don’t miss this place!

3 pizzas came out to $65 ish. Pretty expensive in my opinion especially when there was only one size available and the pizzas aren’t that large (12-13 inch? They seem just a bit larger than a personal pizza)
If you’re really hungry you might want your own pizza, if you’re not too hungry then sharing with one other person might be possible.
I’d go back but I wish they had an option for ordering larger pizzas.

WOW!!!! Absolutely the MOST DELICIOUS AUTHENTIC amazing food in all of my travels this far!! Between the house made Mozerella cheese, fresh homemade pizza crust with fabulous flavor, Pizza sauce is perfect, homemade soup was scrumptious I can’t decide what I loved most! Even the salad was out of this world! Service was also top notch! Fast friendly attentive service! And the atmosphere is cozy & nice. Love this place!!! Bravo!

My wife and I had the best experience at Antica Forma last week! The food was amazing. The pizza was delicious and fresh, the Alfredo was creamy and delicious, the caprese was fantastic, and the peach pie was the best we’ve ever had! Everybody needs to go to Antica Forma!

The Fried Calamar appetizer was perfect..Clam dish was SPOT ON !! The shrimp, mussels, calamari dish was a bit of a disappointment.
The calamari was OVER COOKED.
Being a chef, I was a bit let down, but I’m sure it was just an off day .
I WANT TO GO BACK & taste some of the pizza.
