Family run and operated, the “Boss,” Mimmo Gambino, opened the doors of Casa Rustia in Smithtown, NY, in 1985. In the past, he had created many fine-dining establishments in New York City. Born and raised in a “small town you’ve never heard of in Palermo, Italy,” as Mimmo Gambino used to say, Mimmo came to New York City to purse the American dream. Focusing on his Sicilan upbringing, he instilled a new culture to the recipes that we still use today.
Old-world Italian cuisine is the draw at this upscale ristorante with rustic accents & fireplace.

Casa Rustica – Updated 2026, Italian Restaurant in Smithtown, NY
Reviews
The lobster flambé is an absolute must-try — unforgettable and perfectly prepared table side, with a show! The chicken parm is classic comfort food done right, and the Australian rack of lamb is nothing short of spectacular.
The entire staff is warm, professional, and treats you like family. It’s rare to find a restaurant where the quality of food and service both remain exceptional year after year, but Casa Rustica delivers every single time.
If you’re looking for authentic Italian cuisine, outstanding quality, and a place that truly cares about its guests, I can’t recommend Casa Rustica enough.
From the moment we stepped in, we were warmly greeted and seated with genuine delight. Our waiter was incredibly attentive, patiently answering our questions about the specials and offering his recommendations on the most popular and tastiest entrees. Within minutes, we were toasting to another Valentine’s Day celebration with perfectly crafted dirty martinis that set the tone for the evening.
The appetizers arrived promptly, and we couldn’t have been happier with our choice of clams and oysters. The seafood was impeccably fresh, as if it had just been plucked from the ocean!
My boyfriends main course, lobster, was an absolute delight. Each bite was bursting with flavor, making it one of the most delicious dishes I’ve ever tasted! For my entree, I ordered my favorite, a classic beet and goat cheese salad, which always leaves me satsfied, and this salad was .
True to their excellent service, our waiter even remembered to wrap up the leftovers for me to enjoy as lunch the following day, saving me from forgetting it as we headed towards our valeted vehicle.
Overall, our dining experience at Cafe Rustica was splendid. The freshness, impeccable service, and cozy atmosphere made it a memorable evening. I can’t wait to return for another dinner and indulge in their exquisite cuisine once again.
Highly recommended!
And they have means huge discount for my child birthday party just for been a costumer
We were seated downstairs in a nice cozy dining room.
Our drinks were perfect as well as the table starters they initially place like the breads, oils, etc.
We started off with their broccoli rabe, a Caesar salad and the mussels. Honestly, the sausages in the mussels dish mixed with corn….some of the best we have ever tasted. I should mention that my wife is almost off the boat Italian and a chef herself. We know great cuisine.
For our entrees, we had the rigatoni nonna and the veal chop T. The portions were bountiful and the taste other worldly. We actually saw a coworker with her husband and friends and they spoke so highly as regulars. We were mad we haven’t been here more…but are def gong back real soon. We ended with a cappochino and some tiramisu.
This is real old school Italian, checking off all the boxes. We would highly recommend and would try absolutely anything on this diverse menu.
My only big complain:
We ordered plain pasta and 1 meatball for our 2 year old daughter. The plate was tiny, maybe 15 units of penne? They charged $12 for that. Ridiculous, considering we spent $100+ in dinner.
The “kids menu” wasn’t listed on the menu, so we assumed that was a normal order of pasta without sauce and 1 meatball.
Definitely not a family friendly restaurant.
